THE CHALLENGE

The Objective: Capturing the Cash-Pay Opportunity

The Situation

Midlothian Dermatology was already a successful, high-volume medical practice trusted for treating skin conditions. However, the business faced a specific utilization gap: their existing medical patients were unaware of the practice’s cosmetic and aesthetic capabilities.

My Goal

My goal was to pivot the brand perception from “strictly clinical” to “comprehensive aesthetic care,” driving growth in high-margin, cash-pay services (Dermal Fillers, Neurotoxins) without alienating the core medical patient base.

The Strategy

The Solution: An Integrated Marketing Approach

To drive this growth, I moved beyond isolated tactics and executed a synchronized strategy across Brand, Digital, and Physical channels.

Visual Sketching

Visual Brand Modernization

I overhauled the dated legacy identity, updating the logo, color palette, and typography. The practice had been using brown and tan. The client wanted something calm and serene, and she had chosen a new watercolor painting for the office, so I built the palette from that painting in complementary blues. This refreshed positioning bridged the gap between clinical authority and modern aesthetic appeal, and it meant the website and the physical space matched.

Digital Re-Positioning icon

Digital Re-Positioning

The client wanted a simple, clean site, so I launched with all services on a single page. Over the next three years I built out dedicated pages for each new device and service as the practice added them. These included RF Microneedling, Plexr Plasma Pen, PRX Derm Perfexion, Dermal Filler, Hair Loss Treatment, and Women’s Hormone Health. I later added a Results Gallery and a live Instagram and Google Review feeds.

Targeted Campaigns & Acquisition

I marketed cosmetic services to their existing patient base to encourage cross-selling, while expanding reach through Meta ads targeting local demographics that matched their ideal aesthetic patient profile.

In-Office Strategy

I implemented strategic signage and staff communication training within the physical practice to prompt inquiries like “I didn’t know you did Botox?” during routine skin checks.

High-Intent Search (SEO)

I shifted the SEO strategy to dominate local queries for aesthetic terms, capturing patients specifically looking for cosmetic procedures rather than just medical exams.

Integrated Monthly Marketing

I ran a monthly promotional calendar across email, social media, in-office signage, a specials page on the website, and a site pop-up. Every channel carried the same monthly theme, so a patient who opened the email, saw the Instagram post, and walked into the office got the same message three times.

Email open rates averaged 51 percent, well above the 34 to 40 percent typical for healthcare practices. Instagram engagement ran 3 to 13 percent against a 1 to 3 percent norm for aesthetic accounts. Seasonal campaigns pulled hardest. The Valentine’s Day raffle drew 414 email clicks and 1,464 visits to its landing page.

Over the years the monthly specials page drew 8,943 pageviews. It became the third most visited landing page on the site, behind only the home page and the providers page. During the last year I worked for the practice, it drew 4,277 pageviews, 12.17 percent of all site pageviews.
